<strong>Become a part of our caring community<br><br></strong>We are looking for a dedicated and compassionate Registered Nurse with a Bachelor of Science in Nursing (BSN) to join our team as a Care Manager, Telephonic Nurse 2 within CenterWell Specialty Pharmacy. You will operate where clinicians are engaged on the phone for the duration of their shift, delivering high-quality patient support and education.<br><br>In this important role, you will support our Patient Management Program. This program focuses on individuals with specialized and complex health conditions, including autoimmune disorders, pulmonary diseases, neuromuscular disorders, infectious diseases, cancers, and other rare conditions.<br><br>As a Care Manager, Telephonic Nurse 2, your primary responsibility is to conduct comprehensive telephonic assessments and evaluations of members, with an emphasis on <strong>medication therapy management</strong>. You will provide detailed education on medication administration, potential side effects, and adherence strategies to promote therapeutic outcomes.<br><br><strong>Main Responsibilities Include<br><br></strong><ul><li>Deliver telephonic, patient-centered care in a structured call-center environment throughout the shift</li><li>Provide medication education, including administration techniques, side effect management, and adherence support</li><li>Develop and implement individualized care plans tailored to each member's clinical needs</li><li>Monitor patient progress through ongoing assessments and follow-up interactions</li><li>Collaborate within established protocols and using advanced telecommunication systems to ensure care delivery<br><br></li></ul>You will bring clinical expertise in a structured, phone-based setting. We ask that you have comfort with sustained telephonic engagement and compassionate, patient-focused care.<br><br>We offer the opportunity to improve the lives of patients with complex and rare conditions while working in a supportive and mission-driven environment.<br><br>There is a set schedule that is provided daily with inbound and outbound tasks to perform.<br><br><ul><li>This position is for 10am-6:30pm EST three days a week M-F, and two days a week hours of 11:30am-8pm EST.</li><li>There will be a Friday late night requirement on a rotation of 11:30am-8pm.</li><li>There will be a Saturday rotation hours of 8:30am-12:30pm EST.</li><li>Overtime is required on an as needed basis.</li><li>There is a Holiday rotation that will be worked. Workable holidays for the pharmacy include Martin Luther King Day, Memorial Day, Juneteenth, 4th of July, Labor Day, The day after Thanksgiving, and New Years Day.<br><br></li></ul><strong>Required Qualifications<br><br></strong><strong>Use your skills to make an impact <br><br></strong><ul><li>Bachelors of Science in Nursing (REQUIRED) MUST BE COMPLETED!</li><li>3 - 5 years of clinical acute care experience as an RN.</li><li>Licensed Registered Nurse (RN) in the (appropriate state) with no disciplinary action</li><li>Must live in OH, KY, FL, AZ, TX.</li><li>Comprehensive applications including Word, Excel, and Outlook</li><li>Managed care experience</li><li>Must be passionate about contributing to an organization focused on improving consumer experiences</li><li>Schedule Adherence<br><br></li></ul><strong>Preferred Qualifications<br><br></strong><ul><li>Experience with care management, patient education for adult care, auto dialer experience</li><li>Bilingual English/Spanish</li><li>Auto Dialer Experience<br><br></li></ul>Work at Home Requirements: To ensure Home or Hybrid Home/Office employees’ ability to work effectively, the self-provided internet service of Home or Hybrid Home/Office employees must meet the following criteria: At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is required; wireless, wired cable or DSL connection is suggested. In certain roles, the minimum recommended internet speed required by Humana may not be sufficient for business needs. Humana reserves the right to require associates to upgrade their internet service if necessary. Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information.<br><br>Travel: While this is a remote position, occasional travel to Humana's offices for training or meetings may be required.<br><br><strong>Scheduled Weekly Hours<br><br></strong>40<br><br><strong>Pay Range<br><br></strong>The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.<br><br>$71,100 - $97,800 per year<br><br>This job is eligible for a bonus incentive plan.
